I’m trying to streamline my intake process so I’ve created a new intake form that will help families get started before I send them the plethora of intake forms I need before our evaluation.
Here are the new steps for my intake process:
- Register using the “Register for Treatment” button on the bottom of your screen.
- Complete an intake form. This will expire a month after it’s sent due to high rates of family registration without follow-through.
- Chat with me about your availability. I’ll let you know what openings I currently have, which are coming up, and which I may be able to switch around with current families.
- Complete consent & intake forms. These are required before I provide any services. I’ll run your insurance through my system to make sure I’m in their network so you’re hopefully not landed with a surprise bill. These will expire a week after they’re sent due to high rates of registration without follow-through.
- Schedule our first appointment. Our first appointment will be an evaluation so I can make a case for insurance that they need to pay for services. I’ll also do my best to get to know your child and make them feel comfortable so they won’t dread coming back for treatment.
